What We’re Looking For
Trillium Health Resources has a career opening for a Senior Manager, TCL Transitional Complex Care to join our TCL Complex Transitional Care team. This positions primary responsibilities include providing input for Complex Care Coordination targeting those with chronic, unresolved, or complex physical, behavioral health and social determinant needs. The Senior Manager is responsible for oversight of staff providing proactive intervention care planning and transition to individuals identified to ensure they receive appropriate assessment, discharge/transition planning and services in the community. On a typical day, you might:
- Ensure that Occupational Therapy Staff and Registered Nurses (RN) follow care plan guidelines regarding OT/Nurse evaluation and assessments.
- Assess individuals who may have identified needs with behavioral, physical health, co-occurring, co-morbid or multi-morbid conditions for conditions or issues within the area of occupational therapy that impact transitions from Adult Care Homes to independent living.
- Oversee collaboration with Care Coordinators/Care Managers, integrated care nurse(s), Occupational Therapist(s), COTA(s) and clinician(s) to contribute to integrated care.
- Monitor the Care Plan and Transition plan (physical, behavioral health and social determinant concerns), service delivery and health and safety of the member.
- Oversee completion of occupational and/or nursing assessment and other assessments as needed.
- Build from the resources of the community to assist in linking to services that meet the needs of the members and families within their homes and communities.
- Oversee the evaluation of the member’s environment for hazards or barriers to independent living.
- Oversee identification of assistive equipment needs.
- Oversee the development of Care Plan and Transition Plan Goals for improved functions.
- Participate in Integrated Care Teams staffing.
- Review medical history and assessments to ensure the appropriate linkage to services and supports.
- Provide education regarding functional limitations, visual coordination, written and verbal communication skills, self-care ability, work capacity, adaptive equipment, and treatment options.
- Link members to needed to physical evaluations, occupational therapy assessments, and other specialist(s) as needed.
- Ensure Home Safety Evaluations are completed for members with complex medical and physical needs.
- Ensure the coordination of care with each individual’s primary care physician and/or other identified specialty physician.
- Use independent, professional judgment to oversee complex care coordination/care management functions to assigned population.
- Effectively communicate treatment updates and other member related activities both verbally and in writing to appropriate personnel and agencies.
- Utilize clinical skills and assessment care planning to work autonomously and collaboratively with multiple settings i.e. hospital, school, home, physician office, residential settings, etc.
- Ensure empowerment of members.
- Provide oversight and/or assistance with care transitions from one level of care to another.
- Ensure staff are informed and utilize evidence based practices.
- Oversee staff who assist with Residential Screening Verification Process (RSVP) Screening to determine independent living needs of member.
- Document activities in the Trillium Connections care management platform. Documentation should be timely, accurate, complete, and reflective of the member’s needs.
- Review member information within the Trillium Software platform, Updates information as needed and/or informs the appropriate staff when inaccuracies are found.
- Other duties as assigned.

Qualifications:
Education/Experience:
Bachelor’s degree in Occupational Therapy or Nursing and a minimum of two (2) years documented experience working with the co-occurring of physical health and behavioral/mental health, physical health, or chronic condition population.
OR
Equivalent combination of higher education/experience.
Preferred Education/Experience: Preferably experience in working with BH/MH/SU/IDD population.
Experience in supervision of 2 or more employees preferred
License/Certification:
Must have a valid driver’s license
AND
Fully licensed by the North Carolina Board of Occupational Therapy as an Occupational Therapist.
OR
Fully licensed by the North Carolina State Board of Nursing as a Registered Nurse (RN).
