Fatherhood Class Facilitator/Case Manager

Fatherhood Class Facilitator/Case Manager-

Please note: this position will be primarily located at our Elyria Office on Cleveland St. with some time spent at our Cleveland Office.

Essential Position Functions:

  • Run weekly fatherhood group workshops with participants following evidence based curriculum
  • Assist participants with applying learned skills from the program to their lives
  • Meets with clients (in-person and virtually) 1-2 times per week: individually or in a group to assess needs of the clients; collaborate with clients, families and other case managers and program staff.
  • Process requests for services by conducting intakes, obtaining / completing necessary assessments, completing social histories, and collecting documentation for the determination of participant needs.
  • Completes IDP (Individual Development Plan) for all participants assigned
  • The Fatherhood Coach/Case Manager acts as a liaison between fathers/families and other community agencies.
  • Meet with program staff on a regular basis to ensure appropriate information sharing, coordination of program activities and service delivery
  • Track attendance and all client touch points for program participants in data management systems
  • Assists individuals in accessing community resources.
  • This individual uses strengths based approach to teach and model skills designed to improve family functioning.

Minimum Qualifications:

  • Ability and excitement to facilitate fatherhood workshops during the evening via zoom twice per week
  • Bachelor's degree preferred
  • Experience working with and respect for low-income urban and rural families
  • Experience working with program target population
  • Self starter and go getter who can make successes happen without intensive management
  • Ability to outreach, network, and market participants and program
  • Ability to help participants goal set based on appropriate need prioritizations

  • Proficient in using Microsoft Office and other widely utilized software applications

  • Ability to work well as a team member with colleagues from diverse backgrounds
  • Ability to work independently and meet established deadlines
  • Ability to coordinate a variety of projects simultaneously

Job Type: Full-time

Pay: $39,000.00 - $42,000.00 per year
