Community Program Specialist

The Chronic Disease Control Branch in the California Department of Public Health supports evidence-based intervention programs that promote healthy behaviors, healthy communities, and improve the prevention, diagnosis, and management of chronic disease. Chronic disease prevention aims to ensure a lifetime of wellness for all.

The Health Program Specialist (HPS) I works under the general direction of the Health Program Manager (HPM) II, Program and Policy Section Chief. The HPS I serves as a highly skilled, technical program consultant for Chronic Disease Control Branch (CDCB) programs. The HPS I works collaboratively with an internal multi-disciplinary team and liaises with external partners, such as the Centers for Disease Control and Prevention (CDC). Develops and oversees contractual relationships with health jurisdictions, health systems, and community-based organizations to enhance program strategies and implement program activities to meet performance measures and deliverables. The HPS I coordinates communications, meetings, trainings and technical assistance (TTA), and project meetings. The HPS I serves as a subject matter expert (SME) in team-based care, supporting the engagement of non-physician team members, including pharmacists and community health workers, to promote prevention and management of chronic diseases amongst California’s diverse people and communities, improving health equity and addressing social needs and social determinants of health. In collaboration with program team staff, the HPS I plans, develops, authors, and submits progress reports, semiannual reports, and continuing funding applications to the CDC and other funding organizations. CDCB website team member, researches and responds to drills, legislative inquiries, and public inquires; provides bill analysis; and authors legislative and/or budget proposals as needed. 10% travel in-state and out-of-state is required to attend conferences and trainings.

Desirable Qualifications

In addition to evaluating each candidate's relative ability, as demonstrated by quality and breadth of experience, the following factors will provide the basis for competitively evaluating each candidate:

  • Excellent oral communication and writing skills.
  • Excellent virtual platform skills for meetings, webinars, document sharing, etc.
  • Technical writing and expertise in American Disabilities Act compliance for written documents.
  • Policy and legislative analysis expertise.
  • State budget and contract amendment processes expertise.

Experience in:

  • Effective and professional interactions with internal and external partners.
  • Problem solving and multi-tasking in fast-paced environment.
  • Working with a high degree of independence as well as part of a team.
  • State contract management, budget, and accounting procedures.
  • Federal grant requirements.
  • Contract development and implementation oversight.
  • Event planning and implementation; in-person and virtual.

Knowledge of:

  • Health equity and cultural humility.
  • Program management and clinical services delivery.
  • Public health program design, development, implementation, monitoring, evaluation, and reporting.
