PATH Advocate

Essential Functions:

  • Assess family safety and reporting any concerns regarding the safety of children to supervisor immediately.
  • Schedule families for services in their home and/or in the community which will more than likely take place during non-traditional work hours.
  • Coordinate and facilitate meetings for each family accepted; which will include the family, referral source, foster parents, and other family supports.
  • Provide services weekly to each referred family and completes appropriate parenting training curriculum.
  • Collaborate with referral source to develop a plan in meeting program requirements and services provided for each client.
  • Provide required documentation in accordance with program standards and contract obligations.
  • Meet with referral source personnel regularly to provide ongoing reports of the family’s progress.
  • Facilitate monthly group meetings with community providers and families, as appropriate.
  • Maintain appropriate professional boundaries with clients and families.
  • Assess family’s strengths and needs.
  • Schedule and coordinate visits and transportation of clients and families coordinate visits with foster parents and families which may also include transportation of clients and children.
  • Meet with families to discuss goals, strengths, and opportunities for improvement weekly.
  • Prioritize and target identified risk factors and measure progress for the use during planning meetings.
  • Input information into the electronic databases.
  • Meet 24 hours weekly with clients.

Job Qualifications:

  • Bachelor’s degree in Social Work or related field.
