This is a targeted case manager position which focuses on providing comprehensive case management services to geriatric mentally ill and SPMI adults in a residential treatment facility. Case manager will be expected to conduct assessments, service/treatment plans, prior authorizations, and other clinical documentation. Other functions include conducting outreach, coordinating clients’ care, offering psychoeducation/supportive counseling, and providing service linkage to a maximum caseload of 36. Knowledge of Social Security/Medicare/Medicaid processes preferred. Candidate must be able to drive agency vehicle.

Position Requirements/Qualifications:

  • A minimum of one year experience in a case management like position, working with a consumer population appropriate to the available position is preferred. Experience can be waived for individuals with appropriate field or volunteer experience.
  • Minimum requirement of a Bachelor’s Degree in Social Work, Psychology, or a related human services field from an accredited College or University.
  • Completion of an AHCA approved mental health targeted case management training within three months of hire – Enrollment and payment covered by employer.
  • Attain Florida Certification Board’s Certified Behavioral Health Case Manager Certification within six months of hire – application expense reimbursed by employer.
  • The candidate must be able to demonstrate verbal and written communication skills in the English language. Proficiency in the Spanish language is required.
  • Must pass a level 2 background screening and drug screening.
  • Individual must have a valid Florida Driver’s License with clean driving history and automobile liability coverage as required by Florida law.
