Douglas Gardens CMHC is a non-profit organization that has been providing comprehensive mental health services to the local community for over 40 years.

We are hiring full-time Case Managers for below locations. Case Managers assist clients in meeting mental health and social service needs by advocating, linking, monitoring, and planning for services.

What does a Case Manager do?

  • Conduct case management assessments.
  • Develop treatment plans and case management service plans.
  • Monitor services and progress toward goals, leading to reassessment and development of new service plans or linkages.
  • Intercede on behalf of client to ensure access to needed services and/or resources.
  • Link client to local community resources.
  • Complete prior authorizations for services or medications.
  • Meet with client in off-site locations including client’s home for bi-monthly home visits.

Requirements for Case Managers:

  • Minimum requirement of a bachelor’s degree in the field of human services - Social Work, Psychology, Criminal Justice or a related discipline from an accredited College or University.
  • Minimum one-year of experience in a case management like position, working with a similar client population, preferred.
  • Bilingual English/Spanish.
  • Successfully complete background screening and drug screening.
  • Individual must have a valid Florida Driver’s License and automobile liability coverage as required by Florida law.
  • Completion of AHCA approved mental health targeted case management training within three months of hire – Provided by the company.
  • Attain Florida Certification Board’s Certified Behavioral Health Case Manager Certification within six months of hire –application expense reimbursed by employer.

This is a full-time position, Monday-Friday 8:30am-5:00pm. Benefits to include: Health Insurance, Dental, Vision, Short-term disability, Long-term disability, Life Insurance with optional add-on coverage, 401k with company match, PTO, free parking.
