The Town of Foxborough is seeking a dynamic, customer-service oriented professional to join our team. The Community Health Nurse position is a grant funded position that works on behalf of the Towns of Easton, Foxborough, Mansfield, Norton, Plainville, and Sharon.  This position is based on the availability of Public Health Excellence Grant funds provided through the Department of Public Health. 

Duties include but are not limited to: Organizes community events (e.g., First Aid Training, Narcan Training, Stop-the-Bleed, Mental Health First Aid, and more.);Promotes living a healthy lifestyle and proper self-care throughout the communities served;  Conducts health related outreach throughout the shared services area; Interviews patients to obtain basic data, past medical history, determines income status, and helps facilitate social-services; Supports patients with access to medical appointments. (e.g., helping with tele-med appointments, finding alternative transportation, providing advocacy and advice); Assists with key aspects of patient care, barriers to accessing that care, and to identify potential solutions to those barriers.; Assists clients in finding community services such as, medical assistance, housing assistance, food access, SNAP benefits, and others,; Assists and educates patients with a wide range of medical needs, including but not limited to, tuberculosis, diabetes, heart disease, foodborne illnesses, respiratory illnesses, end of life care.; Maintains cooperative relationships with other communities, community EMS providers, MRC volunteers, and others;  Or other duties, as assigned by the Director of Public Health. 

Recommended Minimum Qualifications and Experience: The successful candidate must be a Registered Nurse (RN).  Candidates who hold a Bachelor of Science (BS) degree in nursing or experience in the public health field will be preferred.   

Salary and Benefits: Full-time (40 hours per week), benefit eligible position. Starting salary is 33.65 dollars per hour.
