Ambulatory Care Clinical Pharmacist

About the Position

We are seeking an Ambulatory Care Clinical Pharmacist to be a part of an interdisciplinary team that provides comprehensive primary care within the Patient Centered Medical Home model. The pharmacist will work under a collaborative practice agreement to manage of patients with chronic illnesses such as hypertension, depression, diabetes, and hepatitis C, etc. The position will provide ongoing education on medications to patients, providers, and clinical staff, as well as train and supervise pharmacy students and PGY1/PGY2 residents.

About the Division

The Community Health Centers of Lane County provide affordable primary care for the whole family – infants, children, adolescents, adults, and seniors – as well as integrated behavioral health, alternative services such as acupuncture, and preventative dental services. At Community Health Centers, we focus on integrated care that values the whole person. Our services encompass not only treatment and prevention of illness, but also health education and behavioral health services.
 
At Community Health Centers of Lane County, we have adopted a model of primary care called Patient Centered Medical Home. Our services are organized around care teams who work collaboratively to support our patients. Our integrated care teams include primary care providers, nurses, medical assistants, patient care coordinators and behavioral health specialists. This model of primary care prioritizes the patient, who has increased access to other services beyond general medical care at our clinics. Our care teams also help patients reach their optimal health status by addressing social determinants of health barriers to care such as transportation, food insecurity, or housing.
 
 *May be eligible for additional bilingual compensation upon completion of a bilingual exam.
 

Schedule: Monday - Friday; 8:00am - 5:00pm

*Possibility of remote work one day a week after training is completed. Start date is flexible.
*Position may include work at any of our locations. Clinic location(s) to be determined at the time of hire.

QUALIFICATIONS:

Training:

  • Graduation from an accredited School of Pharmacy and registration as a Pharmacist by the Oregon State Board of Pharmacy

Experience:

  • One year of pharmacy experience desired.
  • Experience in managed care, disease management, corporate wellness and/or health promotion or equivalent desirable.

Notes:

  • This position is subject to a full criminal offender information record check. 
  • Offers of employment are contingent upon consenting to, and successfully passing, a drug screening test.
  • Please note that as part of the screening process Lane County will verify license and certification status.

Special Requirements:

  • Unrestricted State of Oregon Pharmacy License at the time of hire
  • Successful completion of a pharmacy practice residency or ambulatory care residency, OR 5 years of clinical experience in direct patient care delivery outside of retail pharmacy.
  • Board of Pharmaceutical Specialties certification OR obtain Board of Pharmaceutical Specialties certification within 24 months of hire

  • Current BLS/CPR
  • Note date of birth and social security number when submitting credentialing documents
  • Valid Photo ID must be presented at the Letter of Hire meeting
  • Immunization record and TB test within 10 days of hire
