Social Work Assistant

  • Assisting marginally functioning adults to remain as independent as possible in community settings through assisting social workers who provide services such as, case management, service coordination, and guidance to marginally functioning clients, which includes clients who are receiving voluntary services and clients who the Department serves as Guardian of the Person.
  • Responding to crisis situations and ensuring any emergencies are appropriately addressed.

  • Serving adults and our community by providing essential services such as, but not limited to housing, groceries shopping/errands, bill paying and monthly budgets, medication monitoring, medical appointments, the need for emergency services, education on maintaining housing, hygiene education, accessing local community resources and maintaining and obtaining benefits such as SSDI/SSI, VA, and Medicaid.
  • Maintaining adults’ safely in the community and assuring that their basic needs are being met as adults served are vulnerable and have physical and mental health related conditions.

  • Working cohesively with families, community and agency partners to ensure adults are linked to services that will allow them to function at their highest possible level.

RESPONSIBILITIES: 

  • Plan budgets for assigned payee clients and complete purchase orders for payment of client monthly bills
  • Educate, demonstrate, and assist in household management and proper nutrition
  • Assist clients in obtaining medical services and understanding medical recommendations and follow-up needed
  • Teach clients to obtain own medical treatment, medications, and transportation
  • Provide transportation as needed
  • Assist clients in locating suitable affordable housing and furnishings
  • Demonstrate and teach basic skills of daily living and maintenance of those skills
  • Assist social workers with setting long and short term goals and confer with them on clients’ progress towards goals
  • Plan weekly schedules
  • Complete documentation/reports on a weekly and monthly basis in automated system
  • Use of mobile devices, including tablets and phones, in the field

QUALIFICATONS:

  • Bachelor’s degree in Human Services or related field; or
  • Associate degree in Human Services and 1 year of experience in performing casework or other similar tasks; or
  • High school diploma or GED equivalent and 3 years of experience in performing casework or other similar tasks

ADDITIONAL REQUIREMENTS:

  • A valid Driver’s license with safe driving record
  • Solid computer skills with familiarity in using computers and mobile devices such as tablets and phones
  • Ability to work independently, with strong initiative and flexibility
  • Exceptional communication skills, with the ability to communicate with clients, internal staff, and professionals in the community
  • Ability to work with and teach a diverse population of clients
  • Strong skills in budgeting and home management
  • Knowledge of area resources (medical, nutrition, etc.)
  • This is considered light to medium work; lifting up to 40 pounds
