About the Role
Want to help improve the lives of youth in your community? Seneca is looking for a highly motivated Permanency Social Worker to help youth! As a Permanency Social Worker, you will provide mental health services to youth and their caregivers, who are referred through department of social service department and are transitioning from congregate care to home based care with identified caregivers.
 
The Permanency Social Worker is part of the ASIST Program that provides integrated behavioral health interventions to youth with the goal of supporting youth and caregivers during the transition process. The ASIST program offers resources including support to youth with family finding, coaching to caregivers to support the youth's needs, and in-home mental and behavioral health support services to help youth and caregivers with the transition into a new home. Services will be provided almost exclusively in the community: in families’ homes, group homes, foster homes, schools, etc. The primary goal is to ensure that all parties are working together toward stability for the youth and family.

Responsibilities

  • Meet with clients and caregivers on a weekly basis to further treatment goals.
  • Develop Child and Family teams for the purpose of treatment planning and support.
  • Cultivate and maintain relationships with community members and providers.
  • Coordinate and implement overall treatment and services for assigned clients.
  • Aid in case planning and consultation to team members on clinical issues, as well as casework issues.
  • Intervene with the clients’ behavior management programs.
  • Chart mental health notes, treatment plans, clinical assessments, outcome measures, incident reports, and other documentation accurately and in a timely manner.
  • Participate in crisis response system.
  • Participate in individual and/or group supervision.
  • Function as team leader and uphold the ideals of the program.

Qualifications

Minimum Requirements:
  • Master’s degree in Social Work/Psychology/Counseling.
  • Must be registered (Associates) or licensed with the California BBS.
  • TB test clearance, fingerprint clearances, and all other state/federal licensing or certification requirements.
  • Proof of COVID-19 vaccination.
  • Own vehicle, valid California Drivers License and insurance.
 
Preferred:
  • Experience working with children in out-of-home care.
  • Bilingual in Spanish/English.

  • Full-time

  • Starting at $67,000 per year, with actual salary dependent on creditable experience above the minimum qualifications for the role.
