Case Manager (STAR)

REQUIREMENTS:

 

1.   Has documentation of a minimum of a baccalaureate degree from an accredited college or university, and one year of full time experience in working with adults who have serious mental illness.

2.   Valid FL Driver’s License

3.   Excellent communications skills, verbal/written.

4.   Knowledge of Spanish Language, helpful

5.   Complete the CM Training within three months of hiring date.

6.   One year of experience in Case Management or complete 1 year of employment as Case Manager.

7.   Must meet the minimum requirements to apply for Board Certification.

PRMARY RESPONSIBILITIES:

 

1.   Coordinate outpatient and/or inpatient mental health services for individuals at the when discharged from the Program.

2.   Provide client’s status to supervisor during supervision meetings and demonstrate family involvement as applicable.

3.   Schedule appointments with community mental health providers within 30 days of release.

4.   Follow-up to determine if the individual kept aftercare appointments.

5.   Document client’s related information in the health electronic record.
