Case Manager / Community Health Worker

Overview: 

Pee Dee Healthy Start (PDHS) is seeking a case manager to provide outreach and support activities to pregnant and parenting women, preconception women, and their families to decrease barriers to quality health care throughout seven (7) targeted counties. The Case Manager is a participant/client facing public health team member critical to the Healthy Start team who works directly with individuals and families, has a unique understanding with culture competence as a trusted member of the community in which they serve. The case manager works to establish and maintain trust of the Healthy Start participant which enables them to serve as a liaison between their health and social services providers. The position is federally funded.  

 

Summary:  
Salary:   $28,500 - $38,500   
Status: Full Time

Experience: Relevant experience working with women, infants, children, and families will be considered. Must be able to communicate effectively both oral and in writing. Reliable transportation 
 

Duties and Responsibilities: 

Conducts outreach and recruitment services to identify and enroll pregnant women, their children, and families in targeted Counties. Outreach services includes passing out flyers or other relevant materials by community canvassing (documenting who you spoke with), attendance at community events (in-person and virtual), establishing/maintaining relationships with community/social service organizations, schools, barbershops, hair salons, apartment complexes, housing authority, shelters, local colleges, daycare facilities, county health departments, churches, physician offices/groups, and other relevant community-related entities. 

Additionally, social media platforms can be used to recruit and follow up with Healthy Start participants.   Moreover, the position is responsible for the intake process and complete care coordination needs of assigned participants that can include referrals for services such as mental health, medical, social support, developmental, behavioral health, educational, dental, vision, economic opportunities, career development, job placement and services available and/or needed. 

  
Required Knowledge and Skills: 

Must understand the core services, philosophy, and goals of Healthy Start and the population it serves. Help participants and their families meet their health and social needs by gathering information to understand the participant and their families and provide requisite referrals to connect them to programming, clinical care, and other needed community resources supported by strong follow-up and follow-through mindset. Must have good customer service skills, provide timely documentation of services as well as reliable transportation in working order and a current valid driver’s license.
