Director of Community Care (Hybrid)

Summary

The Director of Community Care is a partner and advisor to the Chief Health Officer and has accountability for the oversight of a broad range of community-facing programs and projects designed to promote the health and wellness of Trenton residents. 

The Director of Community Care will provide oversight and guidance for all community-facing programs across various teams, enabling senior leadership to focus on higher-level organizational and strategic tasks. This person has a deep understanding of community-based care and the unique role of community health workers, and serves as an advocate for their needs across the organization. This role involves oversight of a wide range of programs and direct supervision of program managers.

 

Key Responsibilities

Program Oversight & Leadership: Oversee and manage community-facing programs across Trenton Health Team, providing guidance and direction to program managers

Partner Engagement: Drive deep engagement with providers and community partners to build trust and anticipate needs. 

Community Engagement: Help set the standard for community-centered  care and ensure this standard is consistently met across the organization.

Operational & Financial Management: Balance funding requirements with effective program delivery, discern appropriate overlaps, troubleshoot issues, ensuring funders’ expectations are met alongside effective pursuit of THT’s mission.

Collaboration & Advocacy: Work collaboratively across teams and advocate for the unique role of community health workers across the organization.

Data and Iterative Program Design: Work with the Senior Manager of Data Systems to ensure that appropriate workflows and output measures are configured in Salesforce. Ensure accurate program data is consistently recorded in Salesforce; use data to conduct regular reviews of program effectiveness and support an iterative process of program redesign within teams.

 

QUALIFICATIONS AND EXPERIENCE

A typical strong candidate for this role will have 5-10 years experience of successively responsible project or process leadership roles, either in Trenton, or in communities that experience similar challenges. Directors at THT display the independence of thinking, and critical analytical skills typical of someone with a Masters level education. Candidates should have a minimum of a Bachelors degree in a relevant  field (social work, social policy, public health etc). Experience of programming that supports social determinants of health is an advantage. Clinical qualifications are not required.
